Lifelong's New Welcome Home Meal Delivery Program Gives Low-Income, Isolated Chronically Ill Clients A Fighting Chance to Stay Healthy

Seattle, WA - The days and weeks following a hospital stay are crucial to a person’s recovery. When you are faced with such challenges as lack of family or friends nearby, little to no income, limited mobility, and are living with a persistent, chronic illness — getting better seems so far out of reach.

The first of its kind in the Pacific Northwest, Lifelong’s Welcome Home Meal Delivery Program provides low-income, isolated individuals living with chronic illness with a series of nutritious, tasty meals within 24 hours of release from the hospital. All meals follow guidelines set by the American Diabetes Association and American Heart Association. Meals are customized for people according to their medical diagnosis. For example, the Easy Digestion option is for clients in need of a diet that is gentler on the digestive system, and the Dialysis Care option is for clients in kidney failure.  Meals are packaged to be easily reheated in a microwave or oven.

A growing body of research shows that medically appropriate food is effective in improving health. In 2013, nearly 18% of Medicare patients nationally who had been hospitalized were readmitted within a month.

After the Welcome Home Meal Package is delivered, Lifelong calls the client for regular check-ins. They may ask if the client is taking his/her medications on time or if they need help getting to and from a doctor's appointment. In order to be eligible, you must be living in King County and be coming home from the hospital or nursing facility. Eligibility for services is contingent upon verification of disability due to illness, injury or age. There are no income requirements.

Lifelong accepts referrals from hospitals, emergency responders, or assisted living facilities. The goal is to get you food and get you food quickly. To learn more about the program or to see if you meet the eligibiilty requirements, call (206) 957-1686 or send us a message.
