Learning that you have HIV is a life changing moment. Most people experience a mix of emotions including pain, confusion, fear, anger and loneliness.
You do not have to face this disease alone. Lifelong is here to help people living with HIV/AIDS get connected to the services that can help you live with the disease.
Your first step to receiving services from Lifelong is to meet with an Information & Referral (I & R) case manager. The I & R case manager will discuss your individual needs with you and verify your eligibility for our services.
To set up an appointment with an I & R case manager, please call (206) 957-1717 or e-mail access@llaa.org. You may also drop-in at our Client Services Building, located at 1002 E Seneca St on Seattle’s Capitol Hill.
When coming for your screening, please bring:
• Proof of your income
• Proof of address
• Proof of insurance
• Business cards for your medical providers
• List of any current medications
• Questions you would like answered
Be prepared to answer the following questions:
• Do you currently have an HIV case manager?
• Do you live in King County?
• Where do you receive your medical care?
• Do you have a doctor that can verify your HIV status?
• What needs do you have?
Through answering these questions and completing the screening process, you can help us determine how to best serve you. Since a primary goal of Lifelong is to educate and empower clients, the I & R case manager will also assess your ability to advocate for yourself, complete forms, and complete follow-up steps on your own.
There are several possible outcomes from this screening:
• A referral to HIV case management provider that works with your medical provider, such as Madison Clinic, University of Washington Virology Clinic, Country Doctor Community Clinic, or Pike Market Medical Clinic.
• A referral to HIV case management provider within your local community, such as Pierce County AIDS Foundation or Lifelong in Snohomish County.
• A referral to a Lifelong case manager to complete a comprehensive assessment.
• The I & R case manager has provided you with enough information and guidance during the screening, that you will be able to meet your own needs and no further referrals are needed.
